The Application Form for child assistance benefit (SorPorSor. 201);
- A copy of the spouse’s Citizenship Identity Card
- A copy of the Certificate of Marriage/the Certificate of Divorce of the Insured (in case of divorce registration)
- A copy of the Birth Certificate
- A copy of the first page of the SAVINGS ACCOUNTon which the Account Name and the Account Number are shown (in the case of collecting the benefit through the Bank) by way of 8 commercial banks in which the insured has bank account, namely, Krung Thai Bank PLC; Ayudhya Bank PLC; Bangkok Bank PLC; Siam Commercial Bank PLC; KASIKORN Bank PLC; The Thai Military Bank PLC; Siam City Bank Plc (commencing 1 October 2006) and the Islamic Bank of Thailand (commencing 1 January 2006); by either giving the name of Krung Thai Bank Plc or Bank of Ayudhya Plc. (commencing 1 January 2007).
